The 2009 report that David Lloyd's info comes from (I'm assuming) is posted here: http://www.epa.gov/swerosps/bf/state_tribal/update2009/bf_states_report_2009.pdf.  (My report used an earlier 2006 version of the same data source.)  The raw state-by-state information is in a table starting on p 152.  The two headings are "number of sites in VCP" and "number of sites completing VCP;" so the latter catagory presumably includes sites that have gone through the VCP, but involved no actual cleanup.  NOTE, the asterix clarifies that "VCP" includes other regulatory programs, not just "Voluntary Cleanup."
 
There is no other nationally compiled data that I am aware of; thus nothing that counts unrestricted use, institutional controls, or public involvement.  However, the other columns in the table give some indication of the apples and oranges problem of differenct states counting different types of sites.

Evans,

In your message, you used the term "cleaned up," but your report uses
the phrase "completed state Voluntary Cleanup Programs." Do you have any
idea how many of those sites were "cleaned" to industrial standards, as
opposed to unrestricted use? Might some of these sites have been made
"ready for reuse" without remediating the groundwater, or even putting
groundwater remedies into place (meeting the CERCLA standard of
operating properly and successfully)?

I am not attempting to minimize the value of voluntary cleanups, but I
would like to better understand how well they provide long-term
protection of public health and the environment. The text on page 15-16
of your report does not really answer this question.

Also, in preparing your report, did you make any attempt to determine
how many sites had public notification (of VCP activity), public
disclosure (or suspected or known releases), or public
involvement/oversight of cleanup decision-making?
